ECMO CASE SCENARIOS
Allen
Allen is an 85kg, 48 year-old male with long standing Idiopathic Dilated Cardiomyopathy who presents to the Emergency Department in sustained ventricular tachycardia. After failed attempts at pharmacologic and electrical cardioversion, he is placed urgently on VA ECMO with a 25 French Right Femoral Vein drainage cannula and a 21 French Right Femoral Artery infusion cannula. Support is initiated with a centrifugal pump circuit and PMP membrane, and he is quickly stabilized. Within 2 hours he is awake and neurologically intact, although somewhat anxious.
On day 2 of support, his rhythm has stabilized with multiple anti-arrhythmia medications on board. His mean arterial pressure is 90, he is on no pressors, and he is warm peripherally, with a capillary refill of 2 seconds. His urine output is just over 3cc/kg/hr. He undergoes a sedated TEE which demonstrates that his Ejection Fraction remains reduced at approximately 25%.
