Sharon’s failure to wean from CPB after an acute ischemic event, shock, CPB and cardioplegic arrest, and reperfusion with bypass grafts is consistent with severe myocardial stunning, especially considering her normal pre-event EF, and rapid successful revascularization of her coronaries. With proper “resting” and without the metabolic stress of high-level inotropes, the heart can frequently recover significant cardiac function. Her age of 77 may fall outside some centers age guidelines for ECMO, but if she was a candidate for the cath and the surgery, it would be difficult to deny her a chance at completing the rescue with VA ECMO. Sharon did recover very good function in just a few days of ECMO and was weaned off on ECMO Day #3, and extubated a few days after that. She did require a few days of CVVHD, but actually was off support when she was transferred back to the referring hospital two weeks later to continue her recovery. Two weeks after that she was discharged to home.
