Shirley
Shirley is a 58yo female who is S/P Aorto-Bi-Femoral Bypass for a suprarenal AAA 2 weeks ago. She was extubated on POD 1, but quickly failed and was reintubated. Since then she has been on increased ventilator support including an FiO2 of 100%, maintaining saturations about 90%. Any attempt at weaning the FiO2 or her PEEP, or in waking her up has led to desaturations into the 50% range. She remains sedated and paralyzed. She has remained hemodynamically stable on moderate levels of inotropic suppor, but she has required CVVHD for progression of her chronic renal insufficiency to acute renal failure post-op. You are consulted to evaluate her for ECMO support to allow a decrease of the injurious vent settings.
Additional History - HTN, Hyperlipidemia, Hypothyroidism, chronic renal insufficiency (Cr. 2-2.5) with single functioning right kidney. 100 pack-year TOB Hx, quit 6 weeks ago for this surgery.
Ventilator - RR 30, FiO2 100%, PIP 38, PEEP 12, iNO @ 40ppm.
ABG - 7.30, pCO2 51, pO2 67, Bicarb 25, B.E. -1.5, O2Sat 91%.
