Thomas is a 52yo male who underwent emergency endovascular repair of a type B aortic dissection with visceral ma;perfusion syndrome. Postoperatively he had an elevated WBC count and was cultured and started on antibiotics. By POD#2 he began unexplained hypoxemia with a blood gas of 7.35/44/41/24/-1/73%. With sedation and ventilator management including 100% FiO2 the patient’s saturations were only in the low 80’s. He was tried on 20ppm of iNO without improvement. He was paralyzed and APRV mode with minimal improvement. You are called to evaluate him for ECMO support.

Ventilator - APRV RR 32, P-high 26, P-low 3, T-high 2.5, T-low 0.8, P-mean 20, P-peak 30, FiO2 100%.

ABG - 7.22, pCO2 59, pO2 55, Bicarb 24, B.E. -4, O2Sat 81%.
