Omar
Omar is a 65yo male with a history of tobacco abuse (2ppd), who has been previously healthy and says he has doesn’t take any medicine and has never seen a doctor until 2 days ago when he went to a local clinic because of chest pains. He was diagnosed with atrial fibrillation with a bundle branch block and was transferred to the local hospital. His CPK was 2000 with a Troponin I of 50. He was transferred to your tertiary care center for evaluation and treatment. He was admitted with a diagnosis of acute ST elevation MI with acute on chronic heart failure. He underwent emergency cardiac cath which demonstrated 3 vessel CAD with decreased EF of 15%. While the grafting went well, the patient failed to wean from CPB despite multiple inotropes and an intra-aortic ballon pump (IABP). The surgical team calls you requesting transition to VA ECMO.
