Anthony
Anthony is a 48yo previously healthy male who presented to a local ED with a 3-week history of worsening SOB and fatigue. He was told that he had “cardiac ischemia” and to make an appointment with a cardiologist. Because of worsening symptoms two days later, his wife called EMS and he was brought to your tertiary care center’s ED, where a CTA demonstrated a large left pulmonary artery thrombus and possible multiple right pulmonary artery thrombi. He returned to the ED and was given alteplase (tPA), but shortly thereafter suffered a PEA arrest and CPR and ACLS protocol was initiated. After no response to the first few rounds of drugs, etc, you are called to consider him for ECPR support.

Exam - Fem pulses with CPR. Pupils fixed and dilated.
