James was not offered support because of his multisystem organ failure secondary uncontrolled sepsis and associated rhabdomyolysis. It appeared that his very poor perfusion could not be blamed on high vent settings, although they may contribute to some extent. Placing him on VV ECMO and lowering his vent settings would not likely correct his hypotension, and will not repair the multiorgan dysfunction. He was also deemed to be a poor candidate for sepsis resuscitation on high-flow VA support. The mortality from sepsis induced rhabdomyolysis is very high. He expired the following day.
